WSJ Opinion: Controversial Communication Method for People with Autism

There's a communication method called Rapid Prompting Method, or RPM, and it promises to totally upend everything we think we know about autism. It works by having an autistic person tap out messages on a letter board, which is almost always being held by someone else. The results are miraculous. RPM takes people who are previously assumed to be profoundly intellectually disabled and reveals this hidden intelligence. People using RPM have gone on to graduate college. Some of them have even written books, but this story has a few problems. For one, RPM has a lot of similarities to another long discredited method called facilitated communication. Facilitated communication goes a step further than RPM. It involves a facilitator actually touching the autistic person as he types usually by holding his hand. Facilitated communication also produces the seemingly miraculous results, and it got a glowing treatment from the media at the time despite a lack of scientific validation. The method came under scrutiny in the 1990s after a bunch of kids who were using FC started making allegations of pretty serious abuse. This prompted scientists to rigorously study the method. You know, try to figure out are these kids actually the authors of the messages they're typing? Are they making legitimate allegations? And what these studies found, I mean, it was something like four dozen studies were done on facilitated communication, they all found that it was the facilitator, not the autistic person themselves, who was the true author of the messages that they typed. The result of this is that facilitated communication was widely condemned by major medical associations, and it's now basically regarded as pseudoscience. With RPM, the concern isn't that the facilitator is physically manipulating the user, but instead that she could be subconsciously influencing letter selection by applying interpretive bias when letter selection is unclear, maybe even moving the board, or cuing the user through body language. Unlike facilitated communication, RPM hasn't been subject to any published authorship studies, so there isn't scientific evidence either way. Soma Mukhopadhyay, the woman who invented RPM and brought it to the US in 2001, has historically been pretty hostile to authorship testing. She frames it as insulting. I decided I needed to see RPM for myself, so I hopped on a train from Long Island and observed several sessions with the provider there. What I saw left me conflicted. More than anything, it convinced me that RPM needs to be subject to rigorous scientific inquiry, just like facilitated communication was. If RPM turns out to be legitimate, it could upend the entire scientific consensus about what autism even is. If it turns out to be a case of dressed up facilitated communication, well then that means that a lot of disabled people are being effectively ventriloquized by those who want to help them.
